Applications will be accepted from August 1 through September 1.

If you’re working to address prominent social determinants of health factors like housing, food security, behavioral health services, skilled career training and equitable access to care, we want to hear about your work — and how we can help!

Below, you’ll find all the information you’ll need to apply for funding and partner with us during our next application cycle. We also strongly encourage all potential applicants to view our technical grant support webinar.

Eligibility

  • 501(c)(3) nonprofit organizations and public/governmental agencies are welcome to apply for program implementation funding that will meet identified health and human services needs in Sentara’s Virginia and Eastern North Carolina service areas.
  • Private, nonprofit, or faith-based organizations without 501(c)(3) designations are also eligible to apply for program implementation funding. However, applications must show how the program addresses clearly identified community needs, or extends vital community outreach, and funding requests for private or non-501(c)(3) organizations may not exceed $15,000.
  • While requested funding may support programming that targets specifically identified underserved populations, programming may not exclude members of the population based on race, religion, gender, sexual orientation, or any other affiliation.
  • Organizations must demonstrate a commitment to diversity, equity, and inclusion.
  • The requested funding must support a program or project that strategically aligns with Sentara’s priority areas (outlined below).
  • Therefore, if you are currently implementing a Sentara grant or sponsorship, you may not apply for further funding in the same calendar year.

Funding Priorities

Sentara is focused on supporting organizations and projects that address prominent social determinants of health factors and that promote health equity by eliminating traditional barriers to health and human services. While we will consider proposals that fall outside of the following focus areas, we strongly encourage proposals that align with one or more of the following priorities:

Selection Criteria

Priority will be given to proposals that:

  • Demonstrate clear alignment with one or more of the above priority areas.
  • Demonstrate clear engagement with community partners and stakeholders.
  • Address an identified community need.
  • Address barriers to service delivery to underserved populations or traditionally marginalized communities.
  • Demonstrate the capacity of the organization to reach its impact objectives with measurable outcomes.
  • Demonstrate how the organization plans to leverage matching funding, in-kind support, or other programmatic resources.
  • Contain a clear sustainability plan (if the initiative is projected to continue outside the grant funding period).
